We align our work with CalAIM Community Supports definitions for Housing Transition/Navigation and Housing Tenancy and Sustaining Services.

Housing Transition / Navigation

  • Housing-related needs and barriers assessment.
  • Assistance with applications, documentation, and unit search.
  • Coordination with shelters, interim housing, and permanent housing providers.
  • Move-in planning, including linkage to deposits and furnishings when available.

Housing Tenancy & Sustaining

  • Individualized tenancy support plan with clear goals and responsibilities.
  • Coaching on tenant rights, responsibilities, and house rules.
  • Early identification of tenancy issues and landlord mediation.
  • Ongoing coordination with behavioral health, primary care, and social services.
Benefits & income navigation

Support connecting members to benefits, income, and food resources that stabilize housing.

Landlord engagement

Direct communication with property owners and managers to resolve issues before they escalate.

Crisis tenancy support

Short-term, intensive tenancy support in response to notices, neighbor complaints, or safety concerns.

Who we serve

Medi-Cal members

We work with adults who are homeless, at imminent risk of losing housing, or living in unstable settings, and who are enrolled in participating Medi-Cal managed care plans.

Unhoused or unstably housed High utilization Behavioral health needs

Health plans, providers, and partners

We accept referrals from managed care plans, primary care clinics, hospitals, behavioral health providers, and community-based organizations seeking a focused housing partner.

We provide clear documentation, regular status updates, and coordination with ECM and care teams.

Housing-focused conversations in the community, with attention to safety, preferences, and long-term stability.
Support with paperwork, applications, and coordination so members do not navigate housing alone.

How it works

1. Referral

A health plan, provider, or partner submits a referral with basic information about the member’s housing situation and eligibility for Community Supports.

2. Engagement & assessment

We contact the member, confirm consent, complete a housing and tenancy assessment, and identify immediate risks and priorities.

3. Housing plan & navigation

Together with the member, we create a realistic housing plan and begin navigation: housing search, applications, documentation, and move-in coordination.

4. Ongoing tenancy sustainment

After move-in, we provide regular check-ins, landlord communication support, crisis tenancy interventions, and coordination with the member’s care team to maintain stable housing.
